Prevea Health is pleased to now offer family medicine care in the Appleton region at 2700 E. Enterprise Ave, Suite B. 

Family medicine care is for people of all ages, and in all stages of life – from routine check-ups to vaccinations, and more. Prevea family medicine care in Appleton is provided by  Dana Carroll, APNP, FNP-C

As a family medicine nurse practitioner, Dana provides routine medical care for children and adults, preventive care such as yearly physicals, sports physicals and well-child checks, vaccinations, medication management, sick-visits and treatment of acute injuries.

"My mission is to provide compassionate, holistic and high-quality care for patients and their families at any point in their lifecycle, focusing on their unique needs and health requirements,” said Dana. “I am an advocate for health promotion and disease prevention to help individuals and families live healthy lives."

Appointments for family medicine care in Appleton can be made by calling (920) 272-3610. Dana is currently accepting walk-in patients Monday through Thursday for back-to-school needs such as pre-college and sports physicals. You can also call ahead to get a same day or next-day appointment with no wait.
