Coronavirus Disease 2019 (COVID-19)
COVID-19 vaccinations
- If your child is already scheduled for a visit (such as a well-check) with their pediatrician or other primary care provider, a COVID-19 vaccination can be requested and provided at most appointment.
- If you do not have an appointment already scheduled, please call your Prevea Health pediatrician or other Prevea Health primary care provider directly or use *MyPrevea to schedule an appointment for COVID-19 vaccination.
For those not established with a Prevea Health pediatrician or other primary care provider:
- COVID-19 vaccinations will be available with Prevea Health pediatricians and other primary care providers at Prevea Health locations across Wisconsin, beginning July 6. You do not have to be an established patient with Prevea.
- To make an appointment, you may use *MyPrevea or call 1-833-344-4373.
Dosage and administration of Pfizer COVID-19 vaccine:
- Children ages 6 months to 4 years receive one-tenth the dose used in the adult version
- The first two doses are administered three to eight weeks apart, and a third dose follows at least eight weeks after the second.
Consenting and parent/guardians:
- A parent/guardian must be present for patients presenting for a COVID-19 vaccine (all doses)

- Children 5 through 11 years receive two doses (each one-third of the dose for adults).
- If your child is turning 12 during the three weeks between doses one and two, we recommend waiting to vaccinate until they turn 12 so they can receive the greater dosage. Vaccine dosage does not vary by patient weight but by age on the day of vaccination.
- A parent/guardian must be present for patients ages 5 to 11 years presenting for a COVID-19 vaccine (all doses).

Dosage and administration:
- Pfizer COVID-19 vaccine:
- Adolescents age 12 years and older receive two doses (each the same dosage as adults).
Consenting and parent/guardians:
- A parent/guardian must be present for patients up to 15 years old presenting for a COVID-19 vaccine (all doses).
- If a parent/guardian is not able to be present for a 16 and 17 year old presenting for a COVID-19, the attached consent form must be signed and brought to both doses.
Pfizer vaccine - The Pfizer vaccine is a two-dose series. You will receive your second dose 21 days after your first dose. You are considered fully vaccinated two weeks after receiving your second dose.
Moderna vaccine - The Moderna vaccine is a two-dose series. You will receive your second dose 28 days after your first dose. You are considered fully vaccinated two weeks after receiving your second dose.
Johnson & Johnson vaccine - The Johnson and Johnson vaccine is a single dose vaccine. The Johnson and Johnson vaccine is a single dose vaccine. You are considered fully vaccinated two weeks after receiving your dose. As of May 6, 2022, Prevea has paused administration of Johnson & Johnson vaccine.
Novavax vaccine – The Novavax vaccine is a two-dose series. You will receive your second dose 3-8 weeks after your first dose. You are considered fully vaccinated AND up to date two weeks after final dose, since a booster is not recommended at this time.
Following the approval and recommendation of Centers for Disease Control (CDC) and the Wisconsin Department of Health Services, Prevea is offering COVID-19 booster shots for everyone ages 5 and older.
People ages 5 and older are eligible to receive the updated (also known as “bivalent”) boosters, which are the first Omicron-specific vaccines to be made available in the United States. These boosters target the BA.4 and BA.5 Omicron subvariants, in addition to the original strain of COVID-19.
Prevea will begin administering updated Pfizer boosters to children ages 5 and older, beginning Monday, Oct. 31.

- If you received Pfizer, are age 5+ and moderately or severely immunocompromised, you should get an additional primary shot of Pfizer, given 28 days after 2nd shot.
- If you received Moderna, are age 18+ and moderately or severely immunocompromised, you should get an additional primary shot of Moderna, given 28 days after 2nd shot.
- If you received J&J, are age 18+ and moderately or severely immunocompromised, you should get an additional shot of either Pfizer or Moderna, given 28 days after initial shot
