Green Bay – As vaccination efforts to put an end to the COVID-19 pandemic continue, Prevea Health is taking another step forward in making receiving a COVID-19 vaccination as easy and convenient as possible.
In addition to the Prevea Community COVID-19 Vaccination Clinic at UW-Green Bay’s Kress Events Center, Prevea is now offering COVID-19 vaccination at the following locations:
- Prevea Howard Health Center, 2793 Lineville Rd., Howard
- Prevea Allouez Health Center, 1821 S. Webster Ave., Allouez
- Prevea St. Mary’s Health Center, 1860 Shawano Ave., Green Bay
- Prevea Shawano Avenue Health Center, 1727 Shawano Ave., Green Bay
- Prevea Ashwaubenon Health Center, 2502 S. Ashland Ave., Ashwaubenon
- Prevea East Mason Health Center, 3021 Voyager Dr., Green Bay
- Prevea West De Pere Health Center, 1986 Eisenhower Rd., De Pere
- Prevea East De Pere Health Center, 3860 Monroe Rd., De Pere

If someone would like to visit one of the health centers for the sole purpose of receiving a COVID-19 vaccination, an appointment will be required. You do not have to be an established patient at Prevea to receive a COVID-19 vaccination at a Prevea location.
The fastest way to schedule an appointment is online, through MyPrevea. Visit www.myprevea.com to set up an account at no cost. MyPrevea is also available as an app on any smartphone or other device. If you do not have access or are unable to use the internet, you may call 1 (833) 344-4373 to schedule an appointment.
In addition, those who present to the above locations for health care unrelated to the COVID-19 vaccine and would like to be vaccinated during their appointment, may do so without having to make a separate appointment for vaccination.
It is important
to note that anyone receiving the Moderna or Pfizer vaccines at the above locations must return to the same location either 21 or 28 days later for their second dose. A patient’s appointment for the second dose will be scheduled during the appointment
for the first dose.
Currently, anyone in Wisconsin age 16 and older is eligible for COVID-19 vaccination.
Prevea Health also continues to accept walk-ins for COVID-19 vaccination at the Prevea Community COVID-19 Vaccination Clinic at UW-Green Bay’s Kress Events Center, located at 2358 Leon Bond Dr. in Green Bay.
