For those who receive the COVID-19 vaccine, the vaccination site will providing you with a CDC COVID-19 Vaccination Record Card. We advise that you keep this card in a safe place and do not laminate the card.
If you lose the card, we are unable to issue a replacement card. However, there are still ways that you are able to provide documentation that you have received the COVID-19 vaccine.

MyPrevea via the MyPrevea app:
- Download and or navigate to the MyPrevea app
- Scroll and tap on My Medical Info
- Log in with MyPrevea credentials
- From the home screen, tap COVID-19
- This is downloadable/exportable and contains Lot, Manufacturer, Date of immunization, and Location given
*Note, this screen will also show any past COVID-19 test results as well.

- Navigate to MyPrevea.com and log in with MyPrevea credentials
- On the menu, choose COVID-19
- This is printable and contains Lot, Manufacturer, Date of immunization, and Location given
